Every August in Winnipeg, the same headline resurfaces: heavy backpacks are wrecking kids' spines. It is a tidy story, and it sells reinforced ergonomic packs by the thousand. The research is more measured — and, for parents, more reassuring. Backpack weight and fit are worth getting right, but the alarm around a "heavy bag" is largely overblown. What matters is not a single scary number on a scale; it is how load is distributed, carried, and managed over time.

How much should a school backpack weigh?

The most-quoted guideline is that a child's backpack should stay under 10 to 15 percent of their bodyweight.1 For a 40 kg student, that is roughly 4 to 6 kg. It is a reasonable target, and a useful one for a quick gut check at the front door. But it is worth knowing where that number comes from: much of it is built on short-term studies measuring posture and comfort, not long-term studies showing that heavier bags cause lasting spinal damage.

Systematic reviews looking at the link between backpack weight and back pain in schoolchildren have generally found weak and inconsistent evidence that pack weight alone predicts back pain.2 Factors like a child's own report of the bag feeling heavy, their overall physical activity, and psychosocial stress track more closely with pain than the raw kilograms do. In other words, the number matters less than we have been told — and the child's experience matters more.

Why fit beats fear

If there is a practical villain in this story, it is poor fit, not weight. A pack slung over one shoulder, worn low near the hips, or loaded so it sways with every step changes how a child moves and how the load is shared. Simple adjustments do most of the work:

  • Use both shoulder straps, snugged so the pack sits against the back rather than hanging away from it.
  • Keep the bottom of the pack near the waistline, not down over the buttocks.
  • Use the hip or chest strap if the bag has one — it transfers load off the shoulders.
  • Pack heavy items closest to the spine and lighter items toward the outside.

These are small changes, but they shift the conversation from "how do I stop my child carrying anything" to "how do I help them carry it well." That reframing is the whole point.

Load is not the enemy — too much too soon is

Here is where a rehab-first perspective helps. In sports medicine we have a saying, drawn from Tim Gabbett's work on training load: there are no bad exercises, only too much too soon.3 The same logic applies to a backpack. Carrying load is not inherently harmful to a growing spine — bodies adapt to the demands placed on them, and a moderate daily load is a mild, repeated demand the tissues can accommodate. Problems arise when load spikes suddenly: the first week of term, a project week when every textbook comes home at once, or a growth spurt that changes a child's proportions faster than their strength catches up.

So rather than fearing the pack, watch the trend. A backpack that has been carried comfortably all year is not a new threat in October. A sudden jump in what a child is hauling — or a new complaint that lines up with a spike — is worth a second look. What about posture while carrying?

Children lean forward under a heavier pack — that is a normal, sensible strategy to keep the combined center of mass over their feet, not a sign of damage.4 Posture adapts to load and then returns to baseline once the load comes off. A momentary lean is not a deformity in the making. If anything, the fixation on "perfect posture" under load does more harm than the load itself, and we have written separately about why posture is a weaker predictor of pain than most people assume.

When a child's back pain is worth attention

Most back discomfort in school-aged kids is transient and benign. It is still reasonable to pay attention if pain is persistent over weeks, wakes a child at night, comes with fever or unexplained weight loss, or is linked to a specific injury. Those features have nothing to do with backpacks and everything to do with ruling out the small number of causes that need medical assessment. When in doubt, a clinician can help sort ordinary aches from anything that warrants closer evaluation.

The takeaway for Winnipeg parents

Aim for a pack in the 10 to 15 percent range because it is a sensible ceiling, not because crossing it is dangerous. Spend more energy on fit than on the scale. Watch for sudden spikes in load rather than the load itself. And resist the seasonal panic — a well-fitted backpack, carried on two shoulders, is a normal part of a healthy, active school year, not a threat to your child's spine.
