Back-to-School Sport Physicals: What They're For

As Manitoba families gear up for fall sport, the pre-participation physical evaluation — the "sport physical" — reappears on the checklist alongside cleats and mouthguards. Most people treat it as a formality: a signature to satisfy a league. It is more purposeful than that, and also more limited. Understanding what the exam is genuinely designed to do, and what it cannot do, helps families get value from it instead of treating it as a rubber stamp.

What a pre-participation exam is designed to screen for

The pre-participation physical evaluation has two long-standing goals: to identify conditions that could put a young athlete at risk during sport, and to detect issues that might be improved before play begins.1 In practice that means a screening exam built around a small number of high-value questions and checks:

  • Cardiac screening. The most serious purpose is to flag features that could signal a risk of a cardiac event during exertion — a personal history of fainting or chest pain with exercise, a family history of sudden death at a young age, or relevant exam findings. This is the domain of a physician, and any cardiac concern belongs with a medical doctor for evaluation.
  • Musculoskeletal review. A history of prior injuries, incompletely rehabilitated problems, or areas of reduced strength or mobility that could predispose an athlete to re-injury.
  • General medical history. Conditions such as asthma, prior concussion, or issues that affect participation and may need a plan in place.

The heart of the exam is really the history. A careful set of questions catches far more than the hands-on portion, which is why the standardized forms lean so heavily on personal and family history.2

What the sport physical does not do

Here is the part that gets lost. A pre-participation exam is a screening tool, not a guarantee. A normal exam does not certify that nothing can go wrong; it means no red flags were found on that day with those tests. Screening exams have real and well-documented limits — they can miss conditions, and they can raise concerns that turn out to be nothing.3 That is the nature of screening, not a failure of the exam.

It is also not a general physical, not a fitness test, and not a diagnosis of every ache an athlete carries. Its job is narrow by design: clear the athlete to participate, flag anything that needs a closer look, and hand off appropriately. Framing it that way keeps expectations honest. A cleared athlete who develops chest pain or fainting mid-season still needs medical assessment — the clearance was never a lifetime pass.

Where cardiac screening belongs

Because the cardiac component carries the highest stakes, it deserves a clear line. Questions about exertional symptoms and family history are part of any thorough pre-participation screen, but the interpretation of those answers, and any decision about further cardiac testing, is a physician's responsibility. If a young athlete has fainted during exercise, has chest pain with exertion, or has a family history of early sudden cardiac death, that belongs with their doctor before the season starts — regardless of what any other screening suggests.

Where movement screening fits

The musculoskeletal portion of a standard sport physical is brief — often a quick pass through major joints. That is enough to catch obvious problems, but it says little about how an athlete actually moves under demand. This is where a structured movement screen adds a different kind of information. Rather than asking "is this joint normal," it asks "does this athlete move well, symmetrically, and with control." Tools built for that purpose look at fundamental movement patterns to identify limitations and asymmetries that a static exam misses.4

A movement screen is not a substitute for the medical clearance exam, and it does not predict injury with certainty — no screen does. What it offers is a baseline: a snapshot of movement quality you can return to if something goes wrong later in the season, and a way to spot the athlete whose old ankle sprain is quietly changing how their whole leg loads. That kinetic-chain view — where a limitation in one region shows up as a problem somewhere else — is central to how we think about athletes. Making the exam worth the visit

Treated as a checkbox, the sport physical is a wasted appointment. Treated well, it is a chance to surface a real cardiac question early, to plan around an old injury before it flares, and to establish a movement baseline for the season ahead. Bring an honest history, raise any exertional symptoms directly with the physician, and use the visit to think about how your young athlete moves — not just whether a form gets signed.
